Tigers News · Fall Athletics Registration, Physicals and Concussion Baselines


Dear Moon Area Parents,

As you know, registration for fall sports is underway and now that we have guidance from the PIAA regarding physicals, we have established our dates for physicals and concussion baselines that we will hold at the high school.  I have met with Allegheny Health Network officials and our athletic trainers and we have developed a plan for how to appropriately social distance for these events utilizing time and physical spacing.  Additionally, we will be temperature screening all that come into the building, they will be required to wear masks and we will be asking the standard health screening questions.  Surfaces will be cleaned between physicals/baselines as well.  We typically have around 400 fall athletes therefore we are encouraging you if possible, to pursue physicals with your own doctor, Med Express or at the AHN Urgent Care at Suburban General Hospital in Bellevue (see information below for details) to reduce the number of student-athletes that need to be seen here at the school.

All registrations for sports are done on-line at https://moonarea.8to18.com/  All forms are completed on line with the exception of sections 5 and 6 of the physical form.  These two pages must be printed out and section 5 needs to be completed and taken with you to your physical along with section 6 which the doctor will complete.  Upon completing your physical, sections 5 & 6 must be turned in to the athletic office prior to 8/17/20.

There will be two dates for physicals at the school, Tuesday, July 21st from 5-9 PM and Saturday, August 1st from 8 AM-noon.  The cost for the physical is $20 and checks can be made out to AHN.  If paying with cash, please bring exactly $20.  To sign up for physicals, please go the link below to reserve your spot:

Your child will also need a concussion baseline test on file in order to participate in practice and to compete. All incoming 7th graders and those that have never participated in a school-based sport will need to get a baseline test.  Baselines are done every two years and if you need one, you will be contacted by our athletic trainers to schedule your child’s baseline.  If you know for sure your child needs one, please go to this link:

Monday – Friday  10am – 9:30 pm

Saturday – Sunday  10am – 5:30

No hours on major holidays….

If you need a sports physical, you can stop in at any time to receive your sports physical from the Urgent Care Center

Please make sure you have all of your paperwork filled out, including the HIPPA/Consent to Treat form.  If you do not have your consent to treat form filled out and signed by a parent or guardian you will be turned away. If your school requires that you fill out paperwork online please print out Section 5 and 6 and bring those with you.
